We collect the name, email, phone, and message you send us, plus the service you asked about. In the portal we show the phone, address, appointments, and messages already on your file. We do not ask for a Social Security number, a card number, or a bank account on this website.

We do not pass any individual's consent to receive SMS from 1st Choice Alliance LLC to any third party. We do not share phone numbers with third parties for the purpose of allowing those third parties to send SMS.

Message frequency varies. Message and data rates may apply. Reply STOP to opt out. Reply HELP for help. Consent is not a condition of purchase.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties.
